Point Pleasant Borough Athletics - Sports Physicals
Point Pleasant Borough students planning to try out for any school-sponsored interscholastic sports team are required to have a physical examination performed by a licensed physician. Students cannot try out or practice for any sport until they have submitted a completed Annual Athletic Pre-Participation Physical Examination Form by the specified date. Part A, the Health History Questionnaire, must be complete by a parent of guardian and presented to the examining provider. Part B must be completed by a licensed physician. Physicals are valid for one year. Parents and students are responsible for ensuring physicals are up to date.
All students must have Interscholastic Parent Consent Form and Healthy History Questionnaire Part A, filled out in it’s entirety by parent prior to having physical examination.
Forms cannot be dated 60 days prior to starting the sport.

Winter Sports: Fall Sports begin November 26, 2012. All paperwork must be submitted to the school nurse or or before the November 7, 2012.
